AGREEMENT TO STOP GETTING LETTERS BY U.S. MAIL Letters from Health-e-Arizona Plus (HEAplus) Letters are sent by U.S. Mail. However, most letters can also be viewed in HEAplus. This includes the following letters: • Letters containing requests for information needed to determine eligibility for public benefits, which include Nutrition Assistance, Medical Assistance, and Cash Assistance programs. • Letters showing decisions about your new or renewal applications for public benefits. • Letters showing decisions about changes that affect your eligibility for public benefits. • Letters about tax credit or cost sharing eligibility. • Other letters about general public information. Since letters can be viewed in HEAplus, you can choose to stop getting letters by U.S. Mail. Please note: We are still required to send you some letters by U.S. Mail. It is important that you report to us when your mailing address changes. Mail returned to us as "undeliverable" may cause your benefits to be stopped. Letter process We will post letters in your account on the HEAplus website. You can choose to have HEAplus send you an e-mail alert, a text message alert, or both, when a letter has been posted to your HEAplus account. (Standard message and data rates may apply when selecting text message alerts.) If you choose to get e-mail or text message alerts, you can also stop getting your letters by U.S. Mail. You must log in to your account at www.healthearizonaplus.gov to read your letters. If you choose to stop getting your letters by U.S. Mail, your letters will be considered delivered once HEAplus has successfully sent an e-mail or text alert. If the HEAplus system finds that an e-mail alert did not get sent to you (undeliverable e-mail message), the e-mail alert option in HEAplus will be turned off. If the HEAplus system finds that a text message alert was not received by you due to an undeliverable situation, the text message alert option in HEAplus will be turned off. When both e-mail and text message alerts have been turned off, any letters posted after that date will be sent by U.S. Mail. It is important that you report to us when your mailing address changes. In order to start getting e-mail or text message alerts again and to stop getting letters by U.S. Mail, you will need to select these options again in your HEAplus account. How to stop getting letters by U.S. Mail: To get alerts and stop getting your letters by U.S. Mail, you must complete all of the following: • Have a current User Name and password with HEAplus. • Update your HEAplus account stating that you do not want to get your letters by U.S. Mail. • Accept the "Agreement to Stop Getting Letters By U.S. Mail." • Choose to have HEAplus send you an e-mail alert, text message alert, or both when a letter is posted to your HEAplus account. o Provide a cell phone number if you would like to get your alerts by text message. o Provide a current e-mail address if you would like to get your alerts by e-mail. o Keep your e-mail address and cell phone number up-to-date in your HEAplus account. • Have a current version of Adobe Acrobat (PDF) Reader® software. You can get Adobe Acrobat (PDF) Reader® free at https://get.adobe.com/reader. Your responsibilities When you choose to stop getting your letters by U.S. Mail, you will be responsible for all the following: • Logging into your HEAplus account and checking for new letters regularly. • Opening and reading your letters regularly. • Keeping your e-mail address and cell phone number up-to-date in your HEAplus account. • Reporting when your mailing address changes (for letters that must be mailed to you). How to get your letters by U.S. Mail again If you want to start getting letters by U.S. Mail again, you can do any of the following: • Log in to your account at www.healthearizonaplus.gov, select "Message Center" on the tool bar, and select "Manage My Alerts and Letters." • Contact Health-e-Arizona Plus Customer Support at 1-855-432-7587 to ask that your letters be sent by U.S. Mail. • Give us a request by U.S. Mail, fax, or in person at any local office location. Make sure to include your name, HEAplus Application ID number, and your current mailing address. Privacy of the information in your HEAplus letters The HEAplus alert will only tell you that a letter is available for you to read in your HEAplus online account, which is password protected. The information shown on the letter in HEAplus is private. HEAplus is not responsible if you allow the letter to be seen by others due to your location, password sharing, or if you have a virus or spyware on your computer or mobile device.
